Description

We are interested in hearing the voices of Tennessee's Direct Support Professionals. Specifically, we will examine their perspectives by asking questions in 5 main areas: demographics, training, aspects of the DSP experience, overall quality of life, and describing life as a DSP in your own words. Please help us gather this important information by distributing the attached flyer and survey link to the DSPs you interact with.

This web-based survey for DSPs is anonymous and should take 15-20 minutes to complete. Their answers will help inform us about Direct Support Professionals experience.

To thank Direct Support Professionals for the time and attention that they devote to this survey, we are also offering an opportunity to participate in an optional gift card raffle. We will randomly select 10 participants to receive a $25 gift card. If they are interested in participating in the raffle, they can select that option at the end of the survey. The survey will guide them to another webpage to enter their contact information for the raffle; the contact information will not be linked to their survey responses.
